WeRISE Care Manager Manual
In Hong Kong there are about 22,000 stroke rehabilitees discharged from hospitals each year. Based on this figure, 60 families will be affected each day. Although stroke rehabilitees are the focus of the community’s services for families of stroke patients, as stroke must affect the whole family, the support for both stroke rehabilitees and their family carers is thus dispensable to stroke rehabilitation.
Hence, Lee Hysan Foundation supported the Sau Po Centre on Ageing, The University of Hong Kong, to develop and implement a three-year scheme called ‘WeRISE: Support Action for Families with Stroke Patients’ in 2016 to support families of stroke patients and assist them in re-integrating into the community. This scheme was also collaborated on by the medical and social service units in Southern, Shatin and Tuen Mun districts,
Through the strategy: Family-Oriented, Medical-Social Collaboration and Led by Social Workers, Collaborated with Volunteers, ‘WeRISE: Support Action for Families with Stroke Patients’ has successfully developed a medical-social collaboration model for supporting families of stroke patients. In addition, in the community we have developed and explored an intervention model led by project managers and collaborated by project volunteers (escorts).
During the three-year scheme period, besides distributing Practical Manual for Families of Stroke Patients to about 3,000 families, we have provided health education and consultation for families of stroke patients and successfully served over 250 families through the case management model, and over 560 hours of case management service and over 650 hours of escort service for stroke rehabilitees and carers with outstanding achievements. Preliminary results show that ‘WeRISE’ is welcomed by stroke rehabilitees, carers, case managers and escorts for improving the role performance of participating families, saving medical expenses, and lowering the re-admission rate.
This manual originates from the ‘WeRISE: Support Action for Families of Stroke Patients’ scheme. Through constant improvement during the three-year service process, we hope to present in detail the service vision, objective and specific techniques of intervention that case managers provide for families of stroke patients for the reference of other organisations providing services for families of stroke patients. To ensure the effectiveness of this intervention service, case managers should understand the vision, objective and basic application of strategies of intervention and endeavour to keep close to the guide and progress to achieve specific activities and contents.
